Part A and Part B are the only parts of Medicare that run on public dollars.
All other Medicare insurance is provided by private insurance companies that have agreed to work with, and comply with all Federal requirements and guidelines.
While Part B, and Part D are optional, both cost more the longer one waits to enroll in them.
If an eligible US citizen opts to not pay their Part B premium, all other private Medicare insurance is unavailable to that citizen. The one exception is Part D. A person with only Part A Medicare is eligible to purchase Part D coverage.

Medicare Part D is private insurance to cover Rx drug costs.
Medicare Advantage is private insurance, often referred to as Part C. It may or may not include prescription drug coverage, but usually bundles other coverages like dental, vision, hearing.
Medical Savings Accounts are high deductible insurance plans coupled with a flexible spending account at Optum Bank.
Medical Savings Accounts, otherwise known as MSA's, are not available in every state. Other restrictions also apply, and they do not include prescription drug coverage.
